Position: Permanent (Part time or Full time)
Salary: £14,517 - £21,517 per annum

The successful Community Health Care Assistant will have a dual role in supporting patients in their own home and providing the team with administrative support. You will be an individual with excellent communication skills both on the telephone and in patients homes and with the ability to deliver Health Care to a high standard. You should be committed to joint working, be forward thinking and have a flexible and enthusiastic approach.
As a Community Healthcare Assistant, you will be working as part of a team reporting to the Senior Manager & your duties would include:
* Helping patients to wash or shower
* Assist patients with their personal care
* Serving food and, if necessary, helping patients to eat
* Making and changing beds
* Turning patients who are confined to bed to avoid pressure sores
* Talking with patients to help them feel less lonely or anxious
* Helping patients with mobility problems to move around
* Helping patients to the toilet, giving out and collecting bedpans
* Taking and recording temperature, pulse and respiration
* Take all precautions to ensure the control of infection is minimised whilst delivering care to the patient.
* Act in such a manner as to satisfy public trust & confidence, to uphold & enhance the good standing and reputation of the organisation.
Qualifications: NVQ Level 2 willing to work towards NVQ3
Skills / Abilities: Good Communication & interpersonal skills, ability to prioritise workload, ability to work without direct supervision in a patients home, ability to use own initiative with direction, flexible & adaptable, maintains a calm & mature attitude in difficult circumstances, ability to work as part of a team & ability to anticipate need, motivated to learn and the ability to work with frequent interruptions, whilst maintaining consistent, effective organisation and attention to detail.
Experience: Minimum of two years previous experience (paid or voluntary) working in a community healthcare environment ideally with palliative care experience, experience in caring for dying patients, Knowledge of care giving. Empathy & understanding of issues encountered by patients with palliative care needs, hygiene, health and safety, techniques for lifting and moving patients, personal care, measuring and recording temperature, pulse, respiration and weight, developing communication
Other Requirements: Ability to work flexible shift patterns e.g. days or nights if requested, reasonable knowledge of Brent & Harrow, positive attitude, car driver.
Desirable: NVQ Level 3 or equivalent in experience, experience of working in a specialist palliative care setting, some knowledge of basic anatomy
